A comprehensive osteoporosis education and management protocol improves compliance to medication and enhances discharge to community in an orthopaedic clinic

Abstract
Introduction: Osteoporotic fragility fractures of the hip, spine and wrist occur frequently in the elderly population. These patients are commonly referred to Orthopaedic surgeons for management of their fractures, presenting an opportunity for concurrent investigation and treatment of osteoporosis.

Objectives: This paper aimed to assess the effectiveness of a comprehensive osteoporosis education and management protocol in improving adherence to osteoporosis treatment in an Orthopaedic surgical clinic.

Methodology: A retrospective review of all patients who were referred to and had treatment initiated for osteoporosis in the orthopaedic surgical clinic from 2008 to 2018. The protocol instituted in 2014, comprised of a series of lectures, standardized investigation, established pharmacological management, follow-up and discharges to community for patients with osteoporosis. The number of patients who required discontinuation of treatment, compliance with medication and discharges to community or other primary physicians were recorded for comparison before and after institution of the protocol.

Results: From 2008 to 2017, a total of 2406 patients were initiated with treatment for osteoporosis. A mean of 247 vs 236 patients were recruited yearly after and prior to initiation of the protocol. Less patients developed sentinel events that required discontinuation of medication (11.3% vs 18.5%). More patients remained compliant to their medication (77.4% vs 67.6%). There were a larger number of patients discharged to the community or their primary physician (40.1% vs 13.7%) at 1 year follow-up.

Conclusion: A standardized osteoporosis education and management protocol improves compliance to medication and increases discharge to the community from Orthopaedic specialist care.
